Ten-year Experience with Endologix Afx Endograft for the Treatment of Aortoiliac Occlusive Disease

Endologix AFX unibody stent graft (AFX) has been shown to be effective for the management of complex aortoiliac occlusive disease (AIOD), as a more minimally invasive option than open repair. Additionally, repair is advantageous compared with kissing iliac stents, maintaining normal morphology. The study examines mid-term and long-term results treatment AIOD. A single-center retrospective review was performed patients treated exclusively AIOD device between 2012 2021. One hundred fifteen (68 females [58.6%]) were included evaluation. Primary, assisted primary, secondary patency rates evaluated. Clinical improvement after procedure assessed using Rutherford classification ankle/brachial index. eleven underwent (68% lifestyle-limiting intermittent claudication 32% critical limb ischemia). Technical success 97%, three unsuccessful passages one rupture subsequent mean duration follow-up 28.2 ± 26.2 months. Complications groin thrombosis (n = 2 [1.8%]), infection 4 [3.6%]), common dissection =1 [0.9%]), thromboembolic event 3 [2.7%]). 30-day mortality 1.8% (2/112). Over first year, stage improved from 3.48 0.70 0.88 1.39 (P < .0001). However, 42 (37.5%) required 63 procedures (48 infrainguinal [76.2%] 15 [23.8%]) over period. During period, AOID associated primary 86 4% at 72 months, primary-assisted 95 3% 108 99 1% survival rate 61 8% 88 months (Fig). This represents most comprehensive series regarding use safe, effective, durable. In addition, given high concomitant PAD, allows maintenance aortic bifurcation, facilitating further endovascular treatment.
